Required Immunizations *Three DPTs, with the last one after the age of 4 *Three Polio, with the last one after the age of 4 *Two MMRs, (The first after age 12 months; the second one must be at least 30 days after the first). *Three Hepatitis B vaccinations *Varicella vaccine or Chicken Pox disease if 12 years old *Td Booster due 10 years after the last DTP/dtaP
